Open to legal residents of the United States who are 18 years of age or older at the time of entry and who are registered attendees of the PCRS 2026 conference.
The following individuals are not eligible:
Employees, contractors, and officers of FH Health Inc. and Bloomic
Immediate family members (spouse, parent, child, sibling) of the above
Individuals residing in jurisdictions where this contest is prohibited by law
The contest begins when the entry form is made available at the Bloomic booth at PCRS 2026 and closes at the end of the PCRS 2026 conference. Entries outside this period will not be accepted.
In the event PCRS 2026 is cancelled, postponed, or significantly altered due to circumstances beyond the sponsor's control (including but not limited to acts of God, public health emergencies, or venue closures), Bloomic reserves the right to cancel, modify, or reschedule the contest at its sole discretion. No compensation will be owed to entrants in such circumstances.
Scan the QR code at the Bloomic booth and complete the entry form. You must provide your first name, last name, work email address, clinic name, and role. Limit of one (1) entry per person. Multiple entries, automated entries, and bulk entries will be disqualified.
One (1) winner will receive a pair of Apple AirPods. Model at Bloomic's discretion. Approximate retail value: $150 USD. No cash alternative. Prize is non-transferable. This contest is not affiliated with, sponsored by, or endorsed by Apple Inc.
One (1) winner selected by random draw from all eligible entries at the close of PCRS 2026. Odds depend on total eligible entries received.
The winner will be contacted by email or, if a phone number was provided, by text message, within 48 hours of the draw. If the winner does not respond within 5 business days, the prize may be forfeited and an alternate winner selected.
By accepting the prize, the winner consents to Bloomic using their first name and clinic affiliation for promotional purposes (website, social media, press) unless the winner opts out in writing at the time of notification. No additional compensation will be provided for such use.
The winner is solely responsible for all federal, state, and local taxes applicable to the prize. Bloomic will not withhold or remit taxes on the winner's behalf. Where required by law, the winner may need to complete tax documentation before the prize is released.
Personal information collected through the entry form will be used for contest administration and, where consent is provided, for marketing communications. All data is handled in accordance with Bloomic's Privacy Policy. You may withdraw consent at any time by contacting [email protected].
Bloomic reserves the right to cancel, suspend, or modify this contest at any time without notice if fraud, technical failure, or any factor impairs its integrity.
By entering, participants release FH Health Inc. and Bloomic from any liability arising from participation or acceptance of the prize
Bloomic's decisions on all contest matters are final and binding.
This contest is void where prohibited by law.
This contest is administered by FH Health Inc., incorporated in Canada, and is open to US residents only. Any disputes will be governed by the laws of the Province of Ontario, Canada, without regard to conflict of law provisions. This contest is subject to all applicable federal, state, and local laws.
